Overview

Barrier Failure Analysis (BFA) is an incident analysis technique for determining the root cause of incidents.  It is one of a class of best-practice incident analysis techniques, along with Tripod Beta and BSCAT, aimed at explaining incidents/accidents as being attributed to various degrees of failure in the organizational barriers, controls, defenses and safeguards propounded by modern incident causation theories such as the Swiss Cheese Model.

Faizal Farid Wajidi

    • Principal Consultant, HSE & Risk Management
    • Over 35 years’ experience, with more than 28 in the oil and gas, petrochemical and chemical process industries
    • Spent more than 30 years in mid/senior management capacity
    • B.Sc (Hon.) in Mechanical Engineering with Mathematics from the University of Reading, United Kingdom (1981)
    • Previously worked with Shell, Schlumberger, ICI Physics & Radioisotope Services Group, Risktec and DNV GL
    • Broad experience in HSE & Risk Management, Environmental Assessments, Nuclear Techniques for process plant diagnostics and for elemental analyses, Health Physics, Petrophysics and Project Engineering/Management
    • Qualified and experienced risk management and incident analysis facilitator having led several workshops for major companies in the oil and gas, petrochemical, road transportation, infrastructure, engineering, oil-well services, maritime and aviation/aerospace sectors in Malaysia, Indonesia and Singapore

Faizal is the first of the Netherlands-based CGE Risk’s partners in the South East Asia region to undergo the Advanced Practitioner level certification, a scheme introduced in early 2020.  He is now a proud bearer of the “Advanced Practitioner” badge.

As South East Asia’s sole participant, Faizal completed an “AuditXP and IncidentXP Train-the-Trainers” online course conducted by CGE Risk in April 2021, the first in a new series of stand-alone courses involving the AuditXP and IncidentXP tools on their own.  He was subsequently certified to “Practitioner” level in May 2021, based on CGE Risk’s certification criteria.
