Overview

Tripod Beta is based on an accident causation theory that was developed jointly by Manchester University in the UK and Leiden University in the Netherlands. It takes its name from the association of the three prime elements: unsafe acts, underlying causes and, the accident itself. It acknowledges that human error usually features as a trigger to accidents but the methodology focuses on the organizational deficiencies that lead to human error.

    • Investigation and analyses to be carried out in a thorough, methodical and verifiable manner.
    • A shorter investigation since the ongoing analyses focuses the investigator’s line of enquiry.

This 2-day course is designed for participants to be aware of the requirements of incident investigation and learn how to build a timeline of events leading to incidents, then use the Tripod Beta methodology to analyze the Agents, Objects and Events in incident analysis. The course is highly interactive, switching regularly between input sessions using a presentation style and group work where participants will be involved in a variety of different activities (discussing, analyzing, applying, comparing, categorizing, etc.).

The course also aims to equip participants with the skills to analyze failed barriers, and to employ IncidentXP software as a platform on which to practise these skills. The software will be provided to each user to install on his/her own laptop, on a 30-day free trial basis.

Learning Outcomes

By the end of the course, the delegate will be able to:

    • Understand the background to human errors and violations;
    • Be able to construct an incident timeline or storyboard in advance of analysis;
    • Understand and apply the Tripod Beta methodology;
    • Be able to analyse causation for barrier failures;
    • Be competent in building Tripod diagrams using the IncidentXP software;
    • Produce recommendations and reporting, and;
    • Be able to link Tripod Beta barrier failures to bowties – Learning from Incidents.

About Expert Trainer

Expert Trainer is the Principal Consultant with a broad range of experience in HSE & Risk Management, Environmental Assessment, Health Physics, Nuclear techniques for process plant diagnosis, Petrophysics and Project engineering/management. He has more than 35 years of professional experience, mainly spent in the major hazard sector.

In the field HSE & Risk Management, he has a wealth of experience in the application of risk management and incident analysis. He has led several risk management and incident analysis workshops for safety regulators and for major companies in the oil and gas, petrochemical, road transportation, infrastructure (co- generation, district cooling, biomass, piped gas, desalination), engineering, oil-well services, maritime and aviation/aerospace sectors in Malaysia, Indonesia and Singapore.

Expert Trainer has previously worked with Shell, Schlumberger, ICI Physics and Radioisotope Services Group, Risktec and DNV GL He graduated from the University of Reading, United Kingdom, with a B.Sc (Hon) degree in Mechanical Engineering and Mathematics.

Expert Trainer is the first of the Netherlands-based CGE Risk’s partners in the South East Asia region to undergo the Advanced Practitioner level certification, a scheme introduced in early 2020. He is now a proud bearer of the “Advanced Practitioner” badge.

As South East Asia’s sole participant, Expert Trainer completed an “AuditXP and IncidentXP Train-the-Trainers” online course conducted by CGE Risk in April 2021, the first in a new series of stand-alone courses involving the AuditXP and IncidentXP tools on their own. He was subsequently certified to “Practitioner” level in May 2021, based on CGE Risk’s certification criteria.
